Does my child need a physical before starting Pre-K?
Yes, a physical is required for students entering NC Pre-K. A dental screening is also required, however, if the provider that does the physical makes notation of a dental assessment, a separate dental screening may not be needed. Your school nurse, preschool teacher, or preschool director will let you know if additional documentation is needed. All health documentation is due within 30 days of your child starting the Pre-K program. If it is not received within that time, you child will be excluded from school until it is received. This is NC law as well as an NC Pre-K program requirement.

Yes, a physical is required for all new kindergarten and out of state transfer students (regardless of grade level). It must be documented on the NC Health Transmittal Form. No other form or printout from the doctor can be accepted. If this is not received within 30 days from when your child starts school, your child will be excluded from school until it is received. This is NC state law (G.S. 130A-152).
Is the mother's copy of the birth certificate sufficient?
No- We are required to have a copy of the state birth certificate bearing the state seal.
